There are a couple steps that need to be taken in order to delete an item entirely from a custom form. 

If the item was just placed and there is a need to delete it, use the Shortcut keystroke Ctrl + Z when the canvas is active. See: Shortcut Keys

This will undo the previous placed item from the canvas entirely and no further action needs to be taken.

In the case where the controls were previously placed on the canvas,first highlight the item(s) that are needed to be deleted. If you are deleting an entire group, make sure every item in that group is selected. An easy way to do that is to Click on on of the items in that grid/list and find the group number in the reference portion of the Properties panel. Now select that same group number from the group drop-down also located in the Properties panel. See:Forms Designer - Group

Press the delete key on the keyboard or go to Edit>Delete.

Save the Form.

It will place the deleted item(s) in the hidden layer. 

Click on the Hidden Layer button to find the deleted item. 

Highlight the item or items again.

Press the delete key again to remove these items from the form permanently. 

Save again. 

Control won't Delete

Note: IF THE ITEMS THAT YOU ARE TRYING TO DELETE REAPPEAR AFTER SAVING THE FORM - It means that you have a form saved to the map that uses these entry fields. Any form that contains this control anywhere on the form (including the hidden layer) would need to be deleted before the entry field can be permanently deleted. In the case that it is not desired to delete all of that form's saved records, then other steps need to be taken to ensure the control(s) will not cause issues with the rest of the form.

If it is not required to keep the data that has been previously saved to a form saved under that field name, then the following steps can be taken:

1st remove the control from the visible layer by either deleting it or disabling the visible property.

Then click on the Visible/Hidden button to access the hidden layer.

Find that control, select it, and delete any formula within the control.(if the control is not there, save your form and then access the hidden layer)

Change the name of the control so it is not being referenced by anything else in the form.

Save form & publish to apply these changes to the form.

These steps will allow you to place another control and assign the deleted name.
