This map provides an easy way of generating a Standard Flood Hazard Determination Form .

According to the description of The Standard Flood Hazard Determination Form on FEMA.gov:

"The Standard Flood Hazard Determination Form (SFHDF) is required for all federally backed loans and is used by lenders to determine the flood risk for their building loans. The SFHDF is authorized by the National Flood Insurance Reform Act of 1994 (NFIRA) and is imposed on lenders by their regulatory entities, not by FEMA. FEMA oversees the National Flood Insurance Program (NFIP) which makes federally administered flood insurance available throughout the United States and is responsible for development, updates and making the form available to users. Implementation of the mandatory flood insurance purchase requirements of the Flood Disaster Protection Act of 1973 and the National Flood Insurance Reform Act of 94, as amended, is the responsibility of the various Federal agencies that regulate lenders. Please contact your regulator or lender to determine their requirements. This form may be locally reproduced."

The FEMA Determination Form contains all the required data of the FEMA Form 086-0-32 and is available in Surety and Surety Pro for use. The form uses various available FEMA layer data to populate much of the information on the form automatically. It is a two-page form that conveniently attaches a Flood Zone map to the data entry form.

The form requires drawing or selecting ONE boundary from the Surety or Surety Pro interface then select the FEMA Determination Form in the Report/Form panel.

Tips when filling out the FEMA Determination Form:
- In order to use the Print or Save functions, the "Determination Date" is required to be fill in. This entry box is on the very bottom right corner of the second page.
- The "Loan/Servicer ID:" box is a list based entry form. This allows the saving of ID numbers in the form itself and even setting a default value for every time the form is opened. Please see How to add, delete, and modify Form lists for more information.
