The Check box is a object that is placed on the form when it is intended that the user can select more than one object or a "choose all that apply" scenario.
Check boxes can be used in forms lists and grids.
Check Box
To place on on the canvas, click the Check Box button on the left panel.
Click in the canvas where you want the first check box to be placed.
Keep clicking on the canvas to keep placing more check boxes and stop when you have a sufficient amount of them.
Click on select to change out of the check box tool.
Highlight all of the check boxes that you have placed and align them in a way that works for your form. See:

Click on each individual check box and replace the text in the text property area to be what the option connected to the check box is.
You can also use the properties panel to alter the appearance of the Check box.
