Scale Bar displays the relative scale in feet of the map. It is important to note that the bar is not going to be any specific size such as 1 or 2 inches. The size is dependent on many factors on the client side of the form generation. See Also:

To add a scale bar to your custom form/map's canvas, click on the Scale Bar Button on the left hand panel in the Forms Designer and then click the canvas to place it in the desired location.
Then modify the size and divisions of the scale bar.
Major Divisions will display the length measurements (feet)
Minor Divisions will just be shorter vertical bars
